933.02 APPLICATION; INSURANCE; DEPOSIT TO MUNICIPALITY.
   (a)   Whoever desires a water service connection or a water service connection extension must apply for a permit at the Village Hall. Application may be made by the owner, contractor or licensed plumber who will perform the work.
   (b)   The contractor and/or plumber shall save harmless the Municipality from the operations, actions and omissions of the contractor and/or plumber, his subcontractors and/or their agents. Public liability insurance shall be in an amount not less than one hundred thousand/three hundred thousand dollars ($100,000/$300,000). Property damage coverage shall be not less than fifty thousand dollars ($50,000). The insurance that is furnished by the contractor and/or plumber shall make provisions that the Municipality be an additional insured and in like amounts.
   (c)   A cash deposit of one hundred fifty dollars ($150.00) shall be required before a permit is issued for a water service connection or a water service connection extension. This deposit will be held for a minimum period of three months. After this period, and upon notification by the home owner, the Municipality shall inspect the site. If the site is in satisfactory condition, the cash deposit, less the inspection fee, shall be returned to the owner. If the site is not in satisfactory condition, the property owner shall be informed of the work that must be performed before the deposit can be returned. After the owner has performed such work, he again must notify the Municipality and another inspection will be made. This procedure may be repeated until the Municipality has approved the work. At this time, the cash deposit shall be returned, less any inspection charges at the rate of ten dollars ($10.00) per inspection. The Municipality shall also have the right to perform such corrective action as it feels is necessary to restore the area if the property owner, after notification, does not perform such action within a reasonable period of time. The cost of such work performed by the Municipality shall also be deducted from the cash deposit before it is returned to the property owner.
   (d)   The property owner must certify that there will be no mixing of the City water with water from any other source of supply. It will be permissible to rearrange existing piping and/or plumbing so that separate systems, not interconnected, are established.
