An application for an amusement device license shall be made to the City Clerk on forms furnished by the City Clerk. The application shall set forth the number of coin-in-the-slot amusement devices for which permission is sought for a particular premises, shall describe the number and type of each different device or machine that the applicant intends to operate, and shall be accompanied by payment of the appropriate fee.
