The certificate required by § 6.12.120 shall contain the following information:
(A) The name and address of the owner or harborer of the vaccinated dog;
(B) The kind of vaccine used and the date of vaccination;
(C) The breed, age, color and sex of the vaccinated dog;
(D) The name, address, telephone number and state certificate number of the veterinarian performing the vaccination;
(E) Such other information as may from time to time be required by the Animal Control Officer.
