Any person may appeal the decision or action of officers of the administrative departments by filing, within ten days after such decision or determination, an application for a hearing by the Commission in the following manner:
(A) Applications shall be on forms provided by the city;
(B) The grounds for requesting a ruling by the Commission shall be specified on the application;
(C) The costs of handling and processing the appeal shall be paid for in advance by the applicant as per schedule established by the Commission.
