Each person desiring a license shall file a written application with the city clerk on forms provided by said clerk stating:
(A) The name, age and address of the applicant in the case of an individual; in the case of a copartnership, the persons entitled to share the profits thereof; and in the case of a corporation for profit, or a club, the date of incorporation, the objects for which it was organized, the names and addresses of the officers and directors and, if a majority in interest of the stock of such corporation is owned by one person or his nominees, the name and address of such person.
(B) The location and description of the premises or place of business which is to be operated under such license, specifying the building and the part of such building, as applicable, sought to be licensed and the nature of entertainment to be provided.
(C) Said application shall be accompanied by the following:
1. The full amount of the license fee required to be paid by this chapter: Twenty five dollars ($25.00).
2. Proof of ownership or lease of the premises described in the application for the license term.
3. Proof of public liability insurance coverage for the license term.
4. Written approval from the fire chief of the fire district in which the licensed premises is located. (Ord. 236, 9-5-1978)
