(A) A person desiring a permit to do tattooing, body piercing, body branding or body carving shall file a written application with the Village Clerk's office on a form to be furnished by the Village Clerk's office.
(B) The applicant shall tender with the application the correct permit fee, as provided in § 113.10, and in addition, furnish the following:
(1) The business address and telephone number where the tattooing, body piercing, body branding or body carving establishment is to be operated.
(2) The following personal information concerning the applicant;
(a) Name, complete residence address and residence telephone number.
(b) The two previous addresses immediately prior to the present address of the applicant.
(c) Written proof of age consisting of a birth certificate.
(d) Height, weight and color of hair of applicant.
(e) Authorization for the village, its agents and employees to seek information and conduct an investigation into the truth of the statements set forth in the application and the qualifications of the applicant for the permit.
(f) Written declaration by the applicant, under penalty of perjury, that the information contained in the application is true and correct, with the application being duly dated and signed in the village.
