§ 111.16 APPLICATION FOR LICENSE.
   (A)   The applicant for a license under this chapter shall file with the Village Clerk a sworn application in writing which shall give the following information:
      (1)   Name and physical description of the applicant.
      (2)   Permanent home and local address of applicant, and in case of transient merchants, the local address from which proposed sales will be made.
      (3)   A brief description of the nature of the business and the goods to be sold.
      (4)   The name and address of the employer, if any.
      (5)   The length of time for which the right to do business is desired.
      (6)   If employer is a corporation, the state of its incorporation, whether it is authorized to do business in the state, and evidence that the corporation has designated a resident agent in the village upon whom legal service may be made and that corporation will be responsible for the acts of its employees in this village.
      (7)   A statement as to whether or not the applicant or any of its employees and agents have been convicted of any crime, misdemeanor or violation of any municipal ordinance, other than a traffic violation, the nature of the offense and the penalty imposed.
      (8)   The names of the last cities or villages, not exceeding three, where applicant has carried on business immediately preceding the date of application and the addresses from which the businesses were conducted in those municipalities.
      (9)   Two photographs of the applicant and such of its employees as will be used in the solicitation, peddling or merchandising, taken within 60 days, immediately prior to the filing of the application, which pictures shall be two inches by two inches, showing the head and shoulders of the applicant, or its agents and employees, in a clean and distinguishing manner.
   (B)   Each applicant for a license under this chapter shall pay to the Village Clerk the sum of $150 to defray the cost of processing such application.
