No access permit shall be issued unless a written application for the same is first submitted to the county building inspector directly. The application shall state the name and mailing address of the applicant, the street address of the place of proposed access, structural plans for the off site improvements proposed, and plot plans showing driveway, building setbacks and road dedication lines.
