There is hereby established a fee for nursing services. The amount of the fee will be the sum of:
(a) One hundred fifty dollars ($150.00) as established by the Middletown Board of Health and Environment in accordance with R.C. § 3709.09.
(b) For individual vaccinations, a fee of twenty dollars ($20.00) per administration will be charged.
