There is hereby established a fee for a certified copy of a vital record or a certification of birth. The amount of the fee will be the sum of:
(a) Eleven dollars and fifty cents ($11.50), as established by the Middletown Board of Health and Environment in accordance with Ohio R.C. 3709.09; and
(b) The fee required for the modernization and automation of the system of vital records in the state, as required by Ohio R.C. 3705.24(B); and
(c) The fee required for the Children’s Trust Fund by Ohio R.C. 3109.14; and
(d) The fee, if any, is required for the Family Violence Prevention Fund. The fee is not presently in effect, but is proposed in legislation before the General Assembly to be included to be included in Ohio R.C. 3705.242(A)(1). If this fee is passed, as presently proposed or in some other manner as part of H.B. 66, it will be imposed in the soonest manner permitted by law, but in no circumstances prior to the effective date of the legislation adopting it.
