§ 1416.03 REGISTRATION APPLICATION AND FEES.
   (a)   The fee for registration of a master HVAC contractor shall be two hundred dollars ($200.00) to be paid at the time of registration. Every person upon registration shall present a copy of their active State license to the City along with evidence that they are insured in the minimum amount as required by the Ohio Construction Industry Examining Board in accordance with Ohio R.C. 4740.06(B)(4). The active state license must name the same company as that listed on the registration application. The proof of insurance shall name the City of Middletown as the certificate holder. The master registration shall specify the name of the person so registered, who shall thereby be authorized to engage in the occupation of a master HVAC contractor.
   (b)   An HVAC journeyman registration shall be issued to any person who has duly made application therefore and has passed the examination with a minimum score of 70%, as prescribed by the City. Persons desiring to take the examination for an HVAC journeyman registration shall make written application therefore, accompanied by a fee of fifty dollars ($50.00). Such fees shall not be refunded, but any applicant failing the examination shall be entitled to one re-examination without fee. Exams must be completed within 90 days of the application date. The HVAC journeyman registration shall specify the name of the person so registered, who shall thereby be authorized to perform work as an HVAC journeyman.
   (c)   An HVAC apprentice registration shall be issued, without examination, to beginners or HVAC apprentices employed by a person registered as a master HVAC Contractor with the City, upon application and payment of the fee of twenty dollars ($20.00). The HVAC apprentice registration shall specify the name of the person so registered, who shall thereby be authorized to perform work as an HVAC apprentice.
