The application for the license required in § 848.01 shall be made in writing to the City Manager and such application shall contain the following information:
(a) The full name of each person making application;
(b) The place where the business is to be carried on;
(c) The number of tables to be used for the public playing of pool or billiards; and
(d) A statement that the business, if licensed, will be operated in all respects in conformity with the laws of the State and the ordinances of the City.
