Each parade permit shall contain the following information:
(a) The date of the parade;
(b) The starting time and the termination time of the parade;
(c) The portions of the streets to be traversed or other public places to be used;
(d) The number of persons and motor vehicles that will be in the parade; and
(e) Such other information as the Chief of Police finds necessary for the enforcement of this chapter.
