(A) Prior to the beginning of any work within the right-of-way, the utility company shall register the work with the city in writing, except emergency work which must be registered within 48 hours of the commencement of work. Where a planned project is known to the utility company three to six months in advance, a preliminary registration shall be filed with the city with a later registration showing the detail unavailable at the time of the preliminary registration.
(B) The city shall provide forms containing the information required hereinabove and any other information deemed necessary in the administration of this subchapter; providing, however, that a utility company may devise its own form if the form contains all of the information called for in the city form. The registration may be mailed or delivered to the city, or sent by facsimile.
