§ 153.12 APPLICATION REQUIREMENT.
   Unless otherwise excepted by this chapter, a permit application must include the following, prepared to meet the requirements of this chapter, in order that the permit application be considered:
   (A)   Stormwater Management Plan (§ 153.31);
   (B)   Maintenance agreement (§ 153.42); and
   (C)   Application fee (§ 153.13).
