Every applicant for any license authorized to be granted hereunder shall set forth on a form, to be furnished by the Town Clerk, the following information:
(A) Name and address, both of residence and of business, of the applicant.
(B) Business or employment of the applicant during the five years next preceding the application.
(C) Age of the applicant.
(D) Such other information as may reasonably be required by the Council to enable it to determine the eligibility of the applicant for the license applied for.
