Applications for sewer service shall be filed with the Fiscal Officer upon a form to be supplied by the Municipality. The application shall state the name of the applicant and the premises to be served. All applications filed after the commencement of the operation of the sewer system shall be accompanied by a fee as provided for in Section 907.05.
