(a) In addition to the requirements of Section 804.03, the taxicab operator's license shall contain a description of the equipment to be used, the number of taxicabs to be operated, a statement that the applicant, if granted a license, will so operate its taxicab business as to be available for service to the public at all hours, and such other facts as may be necessary.
(b) Such applications shall be signed and sworn to by the applicant and filed as a permanent record in the office of the Chief of Police.
