(A) Applications for a poultry housing license shall be made to the village in writing on a license application form provided by the village. Each application shall include a non-refundable application fee of $150, and shall as include the following:
(1) The name, address, and telephone number of the individual;
(2) A description of the material that will be utilized for chicken coop, and chicken run;
(3) The location where the chicken coop and chicken run will be located; and
(4) Identify if electricity will be utilized for either the chicken coop or chicken run.
(B) Any person desiring a poultry housing upon their property shall file an application with the village not less than 45 days prior to the time said person is wishing to establish poultry housing.
