Any person, including an applicant for a massage establishment permit, who engages in the practice of massage, shall file an application for a masseur or masseuse permit with the Director of Buildings upon a form provided by the Director of Buildings, and shall pay a nonrefundable filing fee of twenty-five dollars ($25.00) for an original application and ten dollars ($10.00) for a renewal application, to the Finance Director. The Director shall issue a receipt which shall be attached to such application.
