747.04 CONTENTS OF PERMIT APPLICATIONS.
   The application for a permit to operate a massage establishment shall set forth the exact nature of the massage to be administered, the proposed place of business and the facilities therefor.
   In addition to the foregoing, any applicant for such a permit, including any partner or limited partner of a partnership applicant, any officer or director of a corporate applicant, and any stockholder holding more than two percent of the stock of a corporate applicant, shall furnish the following information:
   (a)   His or her name and address;
   (b)   Written proof that the individual is at least eighteen years of age;
   (c)   All residential addresses for the past three years;
   (d)   The applicant's height, weight and color of eyes and hair;
   (e)   The business, occupation or employment of the applicant for the three years immediately preceding the date of application;
   (f)   The massage or similar business license history of the applicant and a statement as to whether or not such person, in previously operating in this City or State or in another city or state under a license, had such license revoked or suspended, the reason therefor and the business, occupation or employment of the applicant subsequent to such action, suspension or revocation;
   (g)   All criminal or City ordinance violation convictions, forfeitures of bond and pleadings of nolo contendere on all charges, except minor traffic violations, within the last five years;
   (h)   The fingerprints and two front-face portrait photographs of the applicant, at least two inches by two inches in size, taken within thirty days of the date of the application; and
   (i)   If the applicant is a corporation, or a partner of a partnership is a corporation, the name of the corporation, set forth exactly as shown in its articles of incorporation.
