An application for the license required by Section 723.01 shall be made to the Mayor upon forms furnished by such official. Such applications shall state the name and address of the person who seeks to represent himself or herself as an astrologer, fortuneteller, clairvoyant or palmist in the City and shall also contain such other information as the Mayor finds reasonably necessary to determine whether or not a license should be issued.
