An application for the permit required by Section 715.02 shall be made to the Director of Buildings upon forms furnished by such official. Such applications shall state the name and address of the person holding, conducting or in charge of the dance and shall also contain such other information as the Director of Buildings finds reasonably necessary to determine whether or not a permit should be issued.
