703.03 LICENSE APPLICATION; ISSUANCE; DECAL OR INSIGNIA.
   (a)   The applicant for a license under this chapter shall secure the necessary application forms from the Director of Public Safety and shall state thereon the number and type of such devices to be installed, operated or maintained in, at or about the establishment or premises, and shall be charged with the duty of reporting any newly installed device or any substitution made during the licensing period.
   (b)   The Director shall be responsible for the preparation of all forms and/or applications and an insignia or decal-type stamp. Upon investigation by the Director, and upon sufficient evidence as presented by the applicant on the application blank furnished by the Director, the Director shall issue a permit covering one mechanical or electrically-operated amusement device and shall further issue an insignia or decal-type stamp to cover the device contained in the application blank. Such insignia or decal-type stamp shall be placed upon such device by the applicant, and the applicant shall ensure that such insignia or decal-type stamp remains affixed thereto during the licensing period.
