The City of Maumee will submit annual reports to Ohio EPA starting one year after the date Ohio EPA has granted the City of Maumee general permit coverage. The report will include:
(a) The status of the City of Maumee's compliance with permit conditions, an assessment of the appropriateness of the identified BMPs, progress toward achieving the statutory goal of reducing the discharge of pollutants to the Maximum Extent Practicable (MEP), and the measurable goal for each of the minimum control measures;
(b) Results of information collected and analyzed, if any, during the reporting period, including monitoring data used to assess the success of the program at reducing the discharge of pollutants to the MEP;
(c) A summary of the storm water activities the City of Maumee plans to undertake during the next reporting cycle (including an implementation schedule);
(d) Proposed changes to The City of Maumee's Storm Water Management Plan, including any changes to any BMPs or any identified measurable goals that apply to the program elements; and
(e) Notice that the City of Maumee is relying on another government entity to satisfy some of the City of Maumee's permit obligations (if applicable).
