(a) Each applicant for a certificate of registration shall, not less than forty-eight hours prior to the commencement of solicitation furnish the Division of Police with the following information on forms provided by the Municipality.
(1) Name, age, date of birth and social security number of applicant.
(2) Complete permanent and local address of applicant.
(3) Name and address of the person, firm, corporation or association for whom the solicitation is presently being made and any other person, firm, corporation or association for whom the applicant has solicited during the past three years.
(4) A description of the nature of the business and the goods, services or wares to be sold or otherwise sufficient to identify the subject matter of the soliciting in which the applicant will engage.
(5) The names of all other municipalities in which the applicant has conducted soliciting activities during the past six months.
(6) Whether the applicant has complied with the requirements of Ohio R.C. Chapter 1716 pertaining to charitable solicitations if applicable.
(7) Whether the applicant has ever been denied a license or permit to solicit or had such license or permit revoked, including the time and place of such denial or revocation.
(8) The make, model, year, color and license plate number of automobiles used by the applicant during the period of solicitation within the Municipality and the number of the applicant's driver's license and state of issuance.
(9) Two (2) passport size photographs which have been taken within the past 6 months.
(b) Each application will be evaluated for the following:
(1) Whether the information furnished by the applicant in accordance with subsection (a) is correct;
(2) Whether the applicant proposes to engage in a lawful commercial, professional or charitable enterprise; and
(3) That neither the applicant nor the enterprise in which the applicant proposes to engage constitutes a clear and present danger to the residents of the City.
(c) In conducting the investigation the Division of Police are authorized to run an informal background check to check the applicant's criminal and traffic records.
(d) If the Division of Police determines that the requirements of subsection (b) and (c) have been met and the appropriate license fee has been paid, then the Division of Police shall issue a license to the applicant. The Division of Police shall attach one photograph of the applicant to the license, where it shall remain at all times. The other photo shall remain with the Division of Police.
(e) Any charitable organization is exempt from paying the registration fee set forth herein. The term "charitable organization" includes any patriotic, philanthropic, religious, social service, welfare, benevolent, educational, civil or fraternal organization, either actual or purported.
