(a) Immediate Reports. The driver or operator of a vehicle involved in an accident within the limits of the City resulting in injury to or death of any person, or property damage, shall immediately by the quickest means of communication, either oral or written, give notice of such accident to the Police Division.
(b) Supplemental Reports. The Police Division may require any driver operator of a vehicle involved in an accident of which report must be made as provided in subsection (a) hereof to file supplemental reports whenever the original report is insufficient in the opinion of such Division with reference to the time, the date, the place, the nature of the accident, weather conditions, road conditions, the name and address of the persons and the ownership of the vehicles directly involved.
(c) Other Occupant to Report if Driver Unable. Whenever the driver or operator of a vehicle is physically incapable of making a required accident report and there was another occupant in the vehicle, or attending the same at the time of the accident capable of making a report, such occupant shall make or cause to be made such report.
(d) Effect of Report; Availability. All required accident reports and supplemental reports shall be made without prejudice to the individual so reporting and shall be available for inspection by persons interested upon application to the Director of Public Safety.

(e) Penalty. Whoever violates any provision of this section is guilty of a minor misdemeanor on a first offense; on a second offense within one year after the first offense, the person is guilty of a misdemeanor of the fourth degree; on each subsequent offense within one year after the first offense, the person is guilty of a misdemeanor of the third degree.
