§ 154.37 WRITTEN STATEMENT REQUIRED CONTAINING NAMES AND ADDRESSES OF CONTRACTORS AND SUBCONTRACTORS.
   If permit is granted for the drilling of any well, before the holder thereof shall commence any work upon any block or tract of land within the city for which a permit has been granted, such holder shall file with the City Clerk a written statement containing the names and addresses of the rig building contractor, the grading contractor, the concrete contractor, and the drilling contractor doing any part of the work incident to the drilling of said well. If the holder of the permit performs all of the work through its various employees, the names and addresses of the superintendents and agents in charge thereof shall be furnished to the City Clerk. In addition thereto the holder of such permit shall furnish to the City Clerk the name and address of the superintendent or other person in charge of the premises upon which any such well is drilled or may be producing oil or gas.
