An application for such license shall contain the name of the applicant, the place where the device or machine will be operated, a description by trade name, model number and serial number of each such device, and evidence that the applicant is a responsible person of good character and reputation having a residence or business address in the city, and if the City Clerk shall be satisfied that such applicant is of good character and reputation he shall cause a license to be issued upon the payment of the license fees.
