Any person, firm or corporation desiring to construct a sidewalk within the municipality shall make a written application which shall be filed with the Director of Community Development, and shall contain the following information:
(A) Name and address of applicant;
(B) The legal description of the real estate adjacent to the proposed sidewalk; and
(C) Such other information as the Clerk or the President and Board of Trustees shall require.
