Application for a license required by this subchapter shall be made on forms provided by the Village Clerk and shall contain the following:
(A) If an individual, the name and address of the individual;
(B) If a partnership, the name, residence and business address of each partner;
(C) If a corporation:
(1) The name of the corporation and the date and state of incorporation;
(2) The names and addresses of its principal officers, directors and local representatives; and
(3) Whether authorized to do business in the state.
(D) The location where the business of renting motorcycles will be conducted, and the number of motorcycles that will be maintained on the premises for rental purposes.
