§ 115.05 LICENSE PROCEDURES.
   (A)   The massage therapy clinic license and the massage therapist license shall be valid for a period of not more than two years following the date of issuance.
   (B)   Upon payment of the application fee, submission of all information required, proper inspection and all requirements described herein are met, a license shall be granted immediately. However, should it appear that any applicant has deliberately falsified any information on the application or the applicant fails to cooperate with the Chief of Police during the application process, the license shall not be granted.
   (C)   Any person denied a license pursuant to these provisions may appeal to the Village Board of Trustees in writing, stating reasons why the license should be granted. The Board of Trustees may thereafter grant or deny the license. The Board of Trustees shall have the right to deny any applicant who has ever been convicted of any crime involving a sexual offense or trafficking in controlled substances or who otherwise does not meet the qualifications or requirements set forth in this chapter.
   (D)   All licenses issued hereunder are non-transferable. However, a change of location of a massage therapy clinic may be permitted pursuant to the provisions hereof.
