(A) Any person desiring to obtain a license to operate a massage therapy clinic or school, or to become a licensed massage therapist, shall make application to the Chief of Police. The Chief of Police shall have the right to confirm any of the information asked for, as provided in the application, as well as inspect the clinic site in order to determine that all requirements have been met and that the site is to be utilized for therapeutic massage.
(B) Application for massage therapy clinic or school license shall be accompanied by a license fee of $100. The applicant for licensure of a massage therapy clinic or school shall be the owner or legal agent of the establishment. The owner or legal agent is not required to be a licensed massage therapist, but a licensed massage therapist must be appointed as a principal massage therapist at the time of the application and at all times subsequent thereto.
(C) Application for a massage therapist license shall be accompanied by a license fee of $25.
(D) Application for massage therapist license shall submit the information in the license application, outlined in Appendix A to this chapter.
