3-11-5: APPLICATION REQUIREMENTS:
Applications for registration shall be filed with the City office and accompanied by a registration fee as established in the schedule of fees in section 3-11-8 of this chapter. Such application shall be made at least thirty (30) days prior to the expiration of the registration, and shall include the following:
   A.   Name, street address, city, state, zip code and telephone number of the owner of the rental property.
   B.   Name, street, address, city, state, zip code and telephone number of the owner's agent responsible for the management of the premises of the rental property.
   C.   Legal address of the rental property.
   D.   Occupancy as permitted under the Zoning Ordinance, or as specified in the Certificate of Occupancy.
   E.   Signed statement of owner or owner's agent indicating that the owner is aware of the City's Building Safety Codes, Nuisance, and Zoning (occupancy) Codes and the legal ramifications for knowingly violating said codes.
   F.   Name and address of the registered agent, if the owner is a corporation.
   G.   Name and address of the mortgage holder, if there is a mortgage on the rental property.
   H.   Name and address of any buyer on a Contract for Deed.
The City may require additional information, at any time, to clarify or support the information provided on an application, or with respect to compliance with any other provision of this chapter. (Ord. 889, 6-24-2019)
