Applications for public vehicle licenses shall be made to the Mayor on blanks provided by the Mayor, and shall set forth the name and address of the applicant, the trade name under which he or she does business, the class of each vehicle for which a license is desired, seating capacity, design, color scheme, lettering, marks to be used on the vehicle, and any other information required by the Mayor. If the application is granted, the applicant shall furnish the Mayor, prior to the issuance of the license tag, with information covering the particular vehicle to be licensed and the information shall include the make, state license number, length of time the vehicle has been in use, motor power thereof, and the name and address of the person, firm, or corporation from which the vehicle was purchased, and any other information required by the Mayor pertinent to the issuance of a license.
