A completed application and questionnaire on forms provided by the Commission, together with the reproducible copy and 10 copies of the preliminary plat showing the supplementary information specified in §§ 153.048 through 153.051, inclusive, shall be submitted to the Commission by the second Tuesday of any month along with a fee as established by the Commission.
