5.60.040: REQUIREMENTS AND DUTIES:
   A.   Alarm System Standards: The chief of police of Mapleton may prescribe minimum standards for the construction and maintenance and require inspection and approval of all alarm systems, proprietor alarms and audible alarms installed within the city. All devices shall meet or exceed such standards before licenses or permits may be issued, pursuant to the provisions of this chapter.
   B.   False Alarm Reports Required: A report shall be filed with the Mapleton police chief for each and every false alarm, setting forth the cause of the alarm and remedial action taken. Where the alarm system is serviced by an alarm business, the report shall be made by such alarm business. Where a proprietor alarm is involved the report shall be made by the permittee. Where more than one false alarm is emitted from one alarm system in any three (3) calendar month period, a report shall be made by the subscriber or user of the alarm system. Such report shall contain the following information. All false alarm reports shall be delivered within three (3) days after the occurrence of the false alarm to the Mapleton police chief:

      (2) Approximate time set off (a.m.) (p.m.)
      (3) Was the business open? (Yes) (No)
      (4) Was this a malfunction of the alarm? (Yes) (No)
      (5) Was this an employee error? (Yes) (No)
      (6) If employee, name of employee
      (7) Have employees received instruction on the proper way to set or turn off alarm? (Yes) (No)
      (8) Person giving the instruction
      (9) Was the alarm being serviced at time of signal? (Yes) (No)
      (10) Was the department notified prior to the service of the alarm? (Yes) (No)
      (11) Were other employees present? (Yes) (No)
      (12) What business installed the alarm?
      (13) How long has the alarm been installed?
      (14) When was it last serviced?
      (15) Do you have a service contract?
      (16) What is the power source of the alarm?
      (17) If after closing hours, how long did it take for you to arrive after being notified?
      (18) If the cause was other than disclosed by (4), (5) or (9), state the cause of the false alarm:
   C.   Audible Alarm Requirements: Every person maintaining an audible alarm, which does not contain a mechanical means to effect an automatic turnoff of the alarm within fifteen (15) minutes after it is first activated, shall post a notice containing the names and telephone numbers of persons to be notified to render repairs for service during any hour of the day or night. Such notice shall be posted near the alarm in such a position as to be legible from the ground level adjacent to the building.
   D.   Automatic Turnoff Device: All audible alarms installed within the city after January 1, 1995, shall be equipped with mechanical means to effect an automatic turnoff and reset. Any existing alarm of this type shall be equipped with an automatic turnoff device by not later than January 1, 1995.
   E.   Service Response: Every person, firm or corporation operating under a license or permit issued pursuant to this chapter shall have and keep available at all hours of each day at least one user representative who shall respond within thirty (30) minutes to emergency service calls from any member of the city police and shall render such emergency service as may be reasonably requested by said officer.
   F.   Subscribers To Conform To And Abide By Rules Of Alarm Business: Subscribers to conform to and abide by rules of alarm business. Every alarm subscriber who shall have an alarm system installed, monitored or maintained by an alarm business licensed under this chapter shall conform to and abide by the rules of operation and use as set forth by the alarm business for the proper use, operation and upkeep of their alarm system.
   G.   Alarm System Inspection: Each alarm system shall be inspected annually. A report of such inspection shall be submitted to the chief of police by the alarm business servicing or maintaining such alarm system. Where a proprietor alarm is involved the report shall be made by the permittee. The inspection report shall be submitted concurrent with the application for renewal of license or permit or within sixty (60) days of alarm system installation. (Ord. 94-15)
