An application on oath by a party in interest, which includes an officer of a corporation or a partner, shall be filed by the developer or owner of land proposed to be developed as provided in these Subdivision Regulations and shall contain the following information:
(a) Names, addresses and telephone numbers of the applicant;
(b) Names, addresses and telephone numbers of the applicant's contractors, subcontractors or other tradesmen for the construction of public improvements; and
(c) Names, addresses and telephone numbers of all owners if other than the applicants, including officers and shareholders of corporate owners.
The application shall be in triplicate and shall be filed with the Secretary of the City Planning Commission, the Clerk of Council and the Director of Service or his or her selected representative at least four days prior to the meeting date of the Commission, Council or the committee of Council which is to review the same.
A similar application containing the information required in subsections (a) and (c) hereof shall be filed with any request for a change of zoning of land, except when made by a public official for the purpose of protecting the general zone plan of the City. He or she shall obtain such information and submit it on belief.
