§ 111.21 LICENSE APPLICATION.
   Each person desiring to operate a taxicab shall file an application for a license to operate, with the Chief of Police, plus pay an annual fee of $12 to the Clerk-Treasurer. The license, after issued, shall expire at midnight on December 31 of the year it was issued. If the license is issued after the first day of January, in any year, the annual fee shall be prorated and adjusted as in § 111.04. The application to be filed by any person desiring to operate a taxicab shall show the following information:
   (A)   Name and address of applicant.
   (B)   The residence of applicant during the past six years prior to application.
   (C)   Name and address of employment for two years immediately preceding application.
   (D)   Applicant's experience in automobile operation.
   (E)   Whether or not the applicant uses alcoholic beverages.
   (F)   Convictions of any crime or misdemeanor within the past five years.
   (G)   Applicant's state public passengers chauffeur number.
   (H)   A current photograph of applicant.
   (I)   Any other information that might be contained in the application.
