(A) The application for the license shall contain the following:
(1) The name, address and telephone number of the individual seeking the license;
(2) The name, address and telephone number of the headquarters of the person, organization, partnership, association, corporation or group of any type represented by the individual seeking the license;
(3) The names, addresses and telephone numbers of the principal officers, management or partners of the organization, partnership, association, corporation or group of any type;
(4) The purpose for which the canvassing, peddling or soliciting is to be made; and
(5) A statement to the effect that if a license is granted, it will not be used or represented in any way as an endorsement by the county, or any of its agents, officers, employees, departments or offices.
(B) It shall be unlawful for any person to submit an application for license containing any misleading or untrue statements.
