The license and application for license must specify all applicable information required in 230 ILCS 15/3, “License—Application—Issuance—Restrictions—Persons Ineligible.” The license application must contain a sworn statement attesting to the not-for-profit character of the prospective license organization, signed by the presiding officer and the secretary of that organization; and that the application will be conducted in accordance with 230 ILCS 15/4, “Conduct of Raffles and Poker Runs.” The application shall also designate a person who will supervise the poker run.
