§ 112.32 CLIENT RECORDS.
   So that the body art practitioner can properly evaluate the client’s medical condition for receiving a body art procedure and not violate the client’s rights or confidential medical information, the practitioner shall ask for the information as follows:
   (A)   In order for the body art practitioner to properly evaluate any health risks prior to a procedure, the client should disclose any of the following conditions:
      (1)   Diabetes;
      (2)   History of hemophilia (bleeding);
      (3)   History of skin diseases, skin lesions, or skin sensitivities to soaps, disinfectants, adhesives, and the like;
      (4)   History of allergies or adverse reactions to pigments, dyes, latex, or other skin sensitivities;
      (5)   History of epilepsy, seizures, fainting, or narcolepsy;
      (6)   Any current communicable diseases; and
      (7)   Use of medications such as anticoagulants, including aspirin, which thin the blood and/or interfere with blood clotting.
   (B)   The body art practitioner shall ask the client to sign an informed consent form confirming that the above information was requested. The client shall be asked to disclose any other information that would aid the practitioner in the client’s body art healing process evaluation.
   (C)   Nothing in this section shall be construed to require the practitioner to perform a body art procedure upon a client.
