§ 112.30 PROHIBITIONS.
   (A)   No mobile, temporary, or transient body art establishment shall operate in the county, except with written permission from the County Board of Health.
   (B)   No person, other than a physician, shall perform any tattooing procedure upon a person under the age of 18 [720 ILCS 5/12-10]. Age verification shall be obtained by government issued identification containing a birth date and photograph.
   (C)   No person shall perform any body piercing procedure upon a person under the age of 18 without the parent or legal guardian signing a consent form in the presence of a practitioner. The consent form shall indicate that the parent or legal guardian has read and understood the disclosure warning statement required under § 797.600(c) of the Illinois Department of Public Health’s Body Art Code. Age verification shall be obtained by government issued identification containing a birth date and photograph. Nothing in this section is intended to require a practitioner to perform any body piercing procedure on a person under 18 years of age with parental or guardian consent.
   (D)   Divisions (B) and (C) above are to be posted as to be clearly visible to all clients.
   (E)   A body art practitioner shall refuse service to any person who, in the opinion of the practitioner, is under the influence of alcohol or drugs. A body art practitioner may refuse service to any person, for any reason, at any time.
   (F)   The following practices are hereby prohibited unless performed by a medical doctor licensed by the Illinois State Medical Board: tongue splitting, implants, skin removal, or scarification requiring the use of a scalpel. Dental procedures such as tooth filing, fracturing, removal, or tattooing shall be performed, exclusively, by a dentist licensed by the State of Illinois.
   (G)   There shall be no eating or drinking by anyone in the area where body art is performed, other than the client.
   (H)   No piercings of any area other than the non-cartilaginous portion of the ear by a stud and clasp piercing system are allowed.
   (I)   It is prohibited to own, operate, or solicit business as a body art establishment or body art practitioner without first obtaining all necessary permits and approvals from Illinois Department of Public Health, and the Macon County Health Department, unless exempted by this chapter.
