§ 112.15 BODY ART PRACTITIONER PERMIT REQUIREMENTS.
   (A)   No person shall practice body art procedures in the county without first obtaining a body art practitioner permit from the County Health Department. Only a person who complies with the requirements of this chapter shall be entitled to receive and retain such a permit.
   (B)   The body art practitioner permit must be renewed annually.
   (C)   The body art practitioner permits for all body art practitioners, including all body art apprentices, working at a body art establishment, must be posted so as to be clearly visible to all clients. A valid permit is one that is not suspended, revoked, or expired.
   (D)   Any person desiring to obtain a body art practitioner permit shall make written application on forms provided by the Department. Applications for body art practitioner permits shall include:
      (1)   Name;
      (2)   Date of birth:
      (3)   Sex;
      (4)   Residence address;
      (5)   Mailing address;
      (6)   Phone number:
      (7)   Place(s) of employment as an operator:
      (8)   Training and/or experience;
      (9)   Proof of attendance at a blood-borne pathogen-training program approved by the Department; and
      (10)   Copy of signed declaration or record of Hepatitis B series.
   (E)   Examples of courses approved by the Department would include such courses as Preventing Disease Transmission (American Red Cross) and Blood-Borne Pathogen Training (US OSHA). Training/courses provided by professional body art organizations/associations or by equipment manufacturers may also be submitted to the Department for approval.
   (F)   (1)   Applicants shall possess knowledge of the following subjects:
         (a)   Anatomy;
         (b)   Skin diseases, disorders, and conditions (including diabetes);
         (c)   Infectious disease control including waste disposal, hand washing techniques, sterilization equipment operation and methods, and sanitization/disinfection/sterilization methods and techniques.
      (2)   Evidence that a body art practitioner possesses knowledge of the above subjects may be demonstrated through submission of documentation of attendance/completion of courses approved by the Department.
   (G)   No body art practitioner permit shall be issued unless, following reasonable investigation by the Department, the body art practitioner has demonstrated compliance with the provisions of this section and all other provisions of this chapter.
   (H)   All body art practitioner permits shall be conditional upon continued compliance with the provisions of this section as well as all applicable provisions of this chapter.
