713.01 PERMIT REQUIRED; FEE.
   (a)   Any person, firm or corporation using a vehicle to remove snow from private driveways and/or parking lots in the City for a fee or charge of any kind shall, on or before October 15 of each year, secure a permit from the Department of Police of the City prior to doing any snow removal.
   (b)   Prior to the issuance of each permit, each applicant shall furnish the make, model and serial number of the vehicle to be used; and the name, address and telephone number of the owner and/or operator employed. Each applicant shall pay the sum of twenty- five dollars ($25.00) for such permit for each vehicle, which shall cover the period from October 15 of the year in which it is issued to May 1 of the following year.
   (c)   Prior to the issuance of each permit, each applicant shall furnish, in addition to those requirements set forth in subsection (b) hereof, the commercial license plate number for each vehicle. Proof of insurance or financial responsibility on a form provided by the City and signed by an insurance company or agent thereof, a signed affirmation, in the form provided below, regarding such insurance and further regarding proper emergency lights for each vehicle as provided herein, and a list of the addresses of the properties to be serviced by said applicant which shall be supplemented throughout said permit period.
AFFIRMATION OF FINANCIAL RESPONSIBILITY AND EMERGENCY LIGHTS
I AFFIRM THAT I WILL NOT OPERATE OR PERMIT THE OPERATION OF A VEHICLE FOR WHICH A COMMERCIAL SNOW REMOVAL PERMIT IS ISSUED BY THE CITY OF LYNDHURST UNLESS SAID VEHICLE IS EQUIPPED WITH PROPER EMERGENCY LIGHTS AS REQUIRED BY LAW FOR SUCH VEHICLES AND PROOF OF FINANCIAL RESPONSIBILITY WITH RESPECT TO THAT VEHICLE IS IN EFFECT AND MAINTAINED AS REQUIRED BY THE LAWS OF OHIO.

   (d)   All motor vehicles engaged in snow plowing operations in the City shall have a flashing amber light in prominent display to give warning to passing motorists.
   (e)   Each applicant to whom a permit is issued shall be furnished with a copy of Section 440.06 of the Traffic Code and 660.05 of the General Offenses Code, along with this section covering the requirements of ice and snow removal. The applicant shall also be issued a decal to be placed on the windshield of each vehicle for which a permit is issued.

   (f)   No person shall be eligible for a permit under this section if that person has been convicted of or pleaded guilty or no contest to a criminal charge involving fraud, theft, deception or a similar crime involving the business of snowplowing within the State of Ohio.
