155.09 HOSPITALIZATION.
   (a)   This hospitalization provision shall not be applicable to the following positions: Part-Time policeman, Auxiliary policeman, janitorial employees or part time employees in the Street Department, Utility man or part time employees in the sewage department, part-time secretaries, Fire Chief, Assistant fire chief, volunteer firemen, zoning and building inspector, tax commissioner, mini bus driver and clerk and all other part time positions.
   (b)   Except as provided in Section 155.11, the Village of Lowellville shall pay all co-pays for hospitalization benefits up to two thousand five hundred dollars ($2,500.00) for a single plan and five thousand dollars ($5,000.00) for a family plan for the Full Time Chief of Police, all regular full-time police officers, the full-time employees in the Street Department, the full-time employees in the Sewage Department, and the full-time secretary.

   (c)   (1)   Any full time employee of the Village, who waives hospitalization benefits shall receive a monthly incentive payment of twenty-five percent (25%) of the savings incurred by Village due to said waiver, or one hundred twenty-five dollars ($125.00), whichever is less.
      (2)   Said written waiver must be signed by the employee, and personally delivered to the Clerk Treasurer in order for said waiver to be effective.

   (d)   (1)   If the eligible family of any full time employee of the Village of Lowellville, waives hospitalization benefits said employee shall receive a monthly incentive payment of twenty-five percent (25%) of the savings incurred by the Village of Lowellville due to said waiver, or one hundred twenty-five dollars ($125.00), whichever is less.
      (2)   Said written waiver must be signed by the employee and the employee’s family members eligible to be covered, and personally delivered to the Clerk-Treasurer in order for said waiver to be effective.
