717.02 LICENSE APPLICATION.
   Every person desiring to engage in the business of palmistry, phrenology or the telling of fortunes by any manner whatsoever must file with the City Manager a sworn application in writing, in duplicate, on a form to be furnished or as required by the Manager which shall give the following information:
   (a)    Name, age and physical description of the applicant.
   (b)    Permanent home address and local address.
   (c)    A description of the nature of the business and the address where the business will be carried on.
   (d)    Applicant's former business, occupation or previous experience or qualifications for such palmistry, phrenology, or the telling of fortunes by any manner whatsoever for five years prior to making the application.
   (e)    The names and addresses of at least three citizens of the City, owning title to real estate in the City, as references regarding such previous experience or qualification, and as to the character of the applicant, together with a signed statement of the references certifying their knowledge of the facts.
   (f)   The length of time for which the right to do business is desired.
   (g)    Two copies of a photograph of the applicant, taken within sixty days immediately prior to the date of the filing of the application, which picture shall be not less than two inches by two inches showing the head and shoulders of the applicant in a clear and distinguishing manner.
   (h)    The fingerprints of the applicant and evidence as to the good character and business responsibility of the applicant as will enable an investigator to properly evaluate such character and business responsibility.
   (i)    A statement as to whether or not the applicant has been convicted of any crime, misdemeanor, or violation of any city ordinance, the nature of the offense and the punishment or penalty assessed therefor.
   (j)    A statement by a reputable physician of the City, dated not more than ten days prior to submission of the application, certifying the applicant to be free of contagious, infectious or communicable disease.
   (k)    Proof that applicant is a citizen of the United States.
