(a) Every person described in Section 656.01 shall report, within one week after taking up residence in the City, in a written statement on a form provided by the Police Department and signed by such person, his or her true name and every other name or alias by which he or she is or has been known, a full and complete physical description of himself or herself, the name of each crime described in Section 656.01 of which he or she has been convicted, together with the name of the place where such crime was committed, the name under which he or she was convicted, the year of the conviction therefor, the address of his or her residence in the City and the date of his or her most recent release from incarceration for the commission of a crime described in Section 656.01.
(b) Whoever violates this section is guilty of a misdemeanor of the fourth degree.
