If, upon re-inspection after a reasonable period of time following the issuance of a notice of violation, the condition has not been corrected, the person responsible shall be issued a citation by the Zoning Administrator. Such citation shall:
(a) Be served as provided by Rule 4 of the Ohio Rules of Civil Procedure;
(b) Identify the violation;
(c) State the time, date and place for appearance in court; and
(d) State the amount of the fine payable.
