§ 157.060 CONTENTS OF APPLICATION.
   (A)   Applications for amendments to the Zoning Map shall contain at least the following information and shall be filed with the Zoning Inspector:
      (1)   Name, address, and phone number of applicant;
      (2)   Proposed amending ordinance;
      (3)   Present use;
      (4)   Present zoning district;
      (5)   Proposed use;
      (6)   Proposed zoning district;
      (7)   A vicinity map at a scale approved by the Zoning Inspector showing property lines, roads, existing and proposed zoning, and such other items as the Zoning Inspector may require;
      (8)   A fee as established by Council;
      (9)   Verification by at least one owner of property that all information in the application is true and correct to the best of his knowledge; and
      (10)   A list containing the name and mailing address of all owners of property within 200 feet of the property in question.
   (B)   Applications for amendments proposing to amend, supplement, change or repeal portions of this chapter other than the Zoning Map shall include items (1), (2), (8) and (9) listed above.
